Secondary School Report
Most colleges, as part of the admission application, require school transcripts and teacher recommendations and many include specific forms for these purposes within their online application. For the Common Application, all school forms are automatically accessible to the College Office after you fill out the Common Application (so you do not need to print any of these forms from CommonApp.org to give to the College Office).
The College Office writes the secondary school recommendation, and the author of that letter collects information from your teachers, advisor, coaches and dorm heads so that a well-rounded picture of you is communicated through this recommendation. Ask if you have any questions about this or let us know if there is any additional information that we should have for this letter.
The College Office requires an official request for transcripts to be sent to colleges or scholarship organizations. A specific form is available for this purpose. Please Click Here for the transcript request form.
Every senior is asked to complete a FERPA (Federal Educational Rights and Privacy Act) consent form on Naviance and this waiver becomes a part of the SSR form. The SSR form (including school recommendation), transcript, teacher recommendations and Putney School profile are sent electronically or mailed together to the college (if by mail, along with a "return receipt" postcard).
Keep in mind that while some colleges don't require any special forms, others have other unique procedures. Just let us know so that we can process the pieces appropriately.
A MID-YEAR REPORT (MYR) is available anytime after the fall Project Week.
